SNC
This tool allows you to screen a single name for sanctions against all OIGCN sanction lists
- Search an Individual first name and last name
- Search an entity Vendor or Business name
- Search any record type, e.g. employee, physician/other or vendor
- Databases include: OIG LEIE, GSA SAM, State Medicaid Lists and Registries, OFAC SDN and other Federal Lists, and optional license disciplinary lists (contact OIGCN for a list of your facility's sources for screening).
Submitting a Name
- Please complete additional fields where possible to assist with our investigations, e.g. SSN, birth date, and middle name
- Please include a Record ID for identification, where available
- Please select the type of name to be screened (employee, physician/other, or vendor)
- Include any notes for our investigator, if desired
- Click the blue Submit button to submit the name.
Screening Progress
Check on the progress of your name screening on the STATUS 30 page:
Click on the blue Details link to view screening details and a link to your report.
Screening Results - No Matches
If the name screened does not result in any matches, the link to your screening report will be immediately visible on the page.
Click on the link to download your PDF report. You can also retrieve your report on the Status 30 page, as described above.
Screening Results - With Matches and Investigations
If the name screened results in matches, our OIGCN team will investigate the matches using the additional information provided to us during submission.
You can monitor the progress of our investigations on the Status 30 page. Use the text search tool at the top of the page to filter results so SingleNameCheck (see picture below).
